About the role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The NP practices to the full extent of their education, training, and certification. The NP has prescriptive authority for a panel of patients or program as indicated by their position. Major duties and responsibilities of the position include, but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Supports the needs of veterans through providing care on walk-in basic and/or coverage of paneled provider veterans as needed. Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Monitors and documents the effectiveness of treatment interventions, achievement of institutional quality measures, and performance measures. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assignments may include management of specialty PACT such as Women's Health. Timely documentation of notes and encounter forms necessary in accordance with facility policy, all administrative work should be completed within a timely fashion. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence.
