About the role

About the Position: This position is located at Naval Medical Center with the Directorate of Clinical Support, Pharmacy Division in San Diego, California. This is a Direct Hire Solicitation.

Serve as a Clinical Pharmacist Practitioner facilitating pharmaceutical care services and direct patient care programs within assigned service(s).

Ensure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulated standards.

Maintain collaborative and comprehensive medication therapy management clinics and services in designated areas of practice, under a scope of practice.

Deliver direct care to patients though a variety of modalities and ensure documentation for encounters and clinical decisions in the electronic health records.

Communicate effectively and professionally with patients, and other members of the healthcare team with the goal to educate, promote patient safety, and practice advancement.

Review electronic medical records to monitor for drug therapy, including evaluation for adverse drug reactions, drug-drug and drug food interactions, disease due to iatrogenic cause, medication therapy duplication, or unnecessary polypharmacy.

Identify high-risk patients and implement interventions to improve quality and safety.

Perform drug usage evaluations, quality assurance/ performance improvement activities as required.

FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
