Outreach Coordinator
About the role
Description
The Outreach Coordinator identifies prospective Venture Forthe clients and builds strong relationships with referral partners throughout the community. This position meets with individuals and families, connects them with appropriate medical and social services, and obtains the documentation needed to support the referral process.
Key Responsibilities
- Develop and maintain relationships with hospitals, physician offices, nursing homes, community organizations, and other referral sources.
- Meet with prospective clients and families in community and home-based settings to understand their needs and explain available services.
- Coordinate appropriate medical and social-service referrals and secure all required documentation.
- Serve as a liaison to referral partners, responding to questions and identifying opportunities to improve satisfaction and collaboration.
- Represent Venture Forthe at community events, coalition meetings, presentations, and networking opportunities.
- Identify new outreach opportunities and promote the company’s services to targeted communities and audiences.
- Maintain accurate referral, relationship, and outreach activity records using CRM and data-tracking systems.
- Protect confidential client and company information in accordance with applicable policies.
- Travel regularly throughout the assigned service area, with travel expected up to 80%.
Requirements
Required
- Bachelor’s degree.
- Two to three years of healthcare experience, including marketing, outreach, or servicing referral sources.
- Experience working with health insurance programs and payer requirements.
- Knowledge of healthcare and home-care systems.
- Strong written, verbal, presentation, relationship-building, and organizational skills.
- Proficiency with CRM systems, Microsoft Office, and data-tracking tools.
- Ability to work independently, manage competing priorities, and develop professional community relationships.
- Valid driver’s license and access to reliable personal transportation for daily travel.
Preferred
- Experience with Medicaid Waiver Programs.
- Established healthcare or community referral relationships.
- Experience presenting services to diverse professional and community audiences.
