About the role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. The Registered Nurse - Infertility and IVF Care Coordinator provides patient management, care coordination, and disposition planning for Veterans, for infertility, IVF and urogynecology. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinates care with multiple providers across all levels and sites of care. Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to Veterans. Provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, timely and completed, while tracking the plan of care. Coordinates and facilitates care including contacting the Veteran, facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, and notifying providers of needed follow up care. The scope of this position includes care occurring in Women Veterans Program. Responsibilities include practice management, triage of patient inquiries (including through secure messaging) with communication of care plans, patient appointment coordination, communication of normal and abnormal test results, and tracking of Veterans identified with infertility/IVF conditions to optimize patient compliance with care plans. The IVF/Infertility Care Coordinator will also coordinate obtaining outside medical records to provide continuity of care. The IVF/Infertility Care Coordinator will serve as an advocate for Veterans, work effectively with team members, and will be proactive in enhancing education for self and the Women Veterans Program.
