Registered Nurse - Home Based Primary Care
About the role
This is an OPEN CONTINUOUS ANNOUNCEMENT & will remain open until 10/29/2026 or until filled. Qualified applicants will be referred to hiring managers at regular intervals. Your application will remain active for 3 months. We are seeking a compassionate skilled Registered Nurse (RN) to provide Home-Based Primary Care (HBPC) services. You will plan & deliver competent care in the home of Veterans' with complex needs, improving their quality of life and supporting their long-term health needs.
The HBPC RN duties include, but are not limited to, the following: Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program. Provides case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. Makes home visits to Veterans in the program. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Evaluates outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations. This could occur on an occasional, non-routine basis at the discretion of the supervisor. This is not a 100% telework-eligible position. Virtual: This is not a virtual position. Drivers License Requirement: A valid, unrestricted, state-issued drivers license is required. Must provide a clean driving record report from the DMV during onboarding. Relocation/Recruitment Incentives: Not authorized. Permanent Change of Station (PCS): Not authorized. U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Must pass pre-employment driver's physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP). You may be required to serve a probationary period.
