Nurse (Disease Management)

About the Position: This position is located at 82nd Medical Group, Sheppard AFB, Texas. This is a Direct Hire Solicitation

Provide comprehensive Disease Management (DM) Services, applying and promoting disease management, health promotion, and wellness concepts and strategies to assigned patients and those identified for disease management.

Develop treatment programs to provide advanced level, specialized, and comprehensive professional nursing care.

Perform clinical or diagnostic procedures.

Provide nursing support to clinic functions during base operational readiness exercises/inspections or major events requiring healthcare assets.

Collaborate with healthcare professionals/departments to identify members needing additional education or clinical management of selected diseases.

Generate data through the use of assessments, screening, and other data tools to identify individuals with specific disease states/conditions/or risk.

Orient healthcare teams to the purpose of disease management activities, population health, clinical preventive services, and medical management.
