Concept Rehab is seeking a Physical Therapist (PT) – PRN to join our therapy team in Waterford, MI.

What You'll Do

As a Physical Therapist, you will:

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination and functional independence
• Monitor patient progress and modify treatment plans as needed
• Collaborate with PTAs and other members of the interdisciplinary team
• Educate patients, families and caregivers
• Participate in care planning and discharge planning
• Complete accurate and timely clinical documentation
• Help patients achieve their highest level of function and independence

What We're Looking For

• Current Michigan Physical Therapist license
• Graduate of an accredited Physical Therapy program
• Strong clinical and communication skills
• Patient-centered approach
• Ability to work independently and as part of a team
• Skilled nursing or post-acute rehabilitation experience is a plus
• New graduates are welcome to apply
