Responsibilities

Principal Duties and Responsibilities

  • Provides skilled nursing care to patients in their homes in accordance with physician orders, agency policies, Medicare Conditions of Participation, and applicable regulatory requirements.
  • Performs comprehensive patient assessments, evaluates physical and psychosocial status, identifies patient care needs, and develops, implements, and updates individualized plans of care.
  • Administers medications and treatments within the scope of practice and agency policies, including medication reconciliation and monitoring for effectiveness and adverse reactions.
  • Provides skilled nursing services including, but not limited to, disease management, wound care, infusion therapy, catheter management, drain care, and patient monitoring.
  • Coordinates care with physicians, patients, caregivers, and interdisciplinary team members to promote continuity of care and achieve optimal patient outcomes.
  • Educates patients, families, and caregivers regarding disease processes, medications, treatment plans, safety measures, and self-management techniques to promote independence and adherence to the plan of care.
  • Recognizes changes in patient condition, communicates findings to the appropriate provider, and initiates appropriate interventions within the scope of practice.
  • Completes OASIS assessments, care planning activities, and all required clinical documentation accurately and within established agency timeframes, including documentation in the EMR/EVV system within 24 hours of the visit.
  • Maintains patient confidentiality and protects health information in accordance with HIPAA, agency policies, and regulatory requirements.
  • Adheres to infection prevention, patient safety, and quality standards while maintaining a safe environment for patients, caregivers, and staff.
  • Participates in quality improvement initiatives and supports agency performance goals related to patient outcomes, patient satisfaction, regulatory compliance, and operational excellence.
  • Maintains professional competency through completion of required education, annual competencies, regulatory training, and continuing professional development activities.
  • Manages nursing supplies, equipment, and resources responsibly and in accordance with agency procedures.
  • Performs other duties and responsibilities as assigned in support of agency operations and patient care needs.
  • Qualifications

    Knowledge, Skills and Abilities

    • Knowledge of nursing principles, practices, and standards of care.
    • Ability to perform comprehensive patient assessments and utilize critical thinking and clinical judgment to identify patient care needs and implement appropriate interventions.
    • Ability to administer medications and treatments safely and accurately in accordance with physician orders, agency policies, and nursing standards.
    • Ability to interpret laboratory, diagnostic, and clinical findings and communicate significant changes in patient condition to the appropriate healthcare provider.
    • Proficiency in wound care, infusion therapy, drain management, indwelling catheter care, medication reconciliation, and other skilled nursing interventions.
    • Ability to operate and utilize medical equipment commonly used in patient care.
    • Excellent verbal, written, and interpersonal communication skills with the ability to effectively communicate with patients, families, physicians, and interdisciplinary team members.
    • Ability to provide patient and caregiver education regarding disease processes, medications, treatments, and self-care management.
    • Strong organizational, time management, multitasking, and problem-solving skills with the ability to work independently and manage a patient caseload.
    • Ability to establish and maintain effective working relationships while demonstrating professionalism, compassion, courtesy, and respect.
    • Ability to maintain patient confidentiality and exercise sound professional judgment.
    • Proficiency in electronic medical record (EMR) systems and timely, accurate clinical documentation, including completion of visit documentation within 24 hours of the visit date.

    Education

    Associates of Nursing is Required

    Bachelor's of Nursing is Preferred

    Experience

    1 year experience in Med Surg or Critical Care is Required

    Certifications and Licensures

    Licenced Registered Nurse is Required

    Certified Basic Life Support (CPR) is Required
