About the role

Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates the effectiveness of the Veteran's care. Serves to support delivery of high quality, timely care to the Veteran receiving services in the community. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Duties include but are not limited to: Assists in clinically triaging consults/referrals both internal and those eligible for community care. Assists in reviewing community care documents via fax streem and takes appropriate action to expedite Veterans' clinical needs. Assists in providing Veterans with timely in-network community care referrals. Assist in providing community providers with referrals and authorizations which are consistent with community standards. Serves as a liaison between the referring provider and specialty care services. Has advanced knowledge of community care electronic software, data bases and tools to guide decision making, determine all options of care available based on Veterans' clinical need and review and approve consults. Provide community providers with referrals and authorizations which are consistent with community standards. Other duties as assigned.
