Registered Nurse (Home Based Primary Care)/Outpatient

The Registered Nurse (Homebased Primary Care)/Outpatient provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Homebased Primary Care (HBPC) Program. NOTE: The 2-page Resume requirement does not apply to this position.

The Registered Nurse (Homebased Primary Care) is responsible for providing competent, evidence-based care to HBPC patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. Other duties/responsibilities include but are not limited to the following: Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. Conducts home visits to Veterans in the HBPC program. Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Demonstrates leadership in delivering and improving holistic, home-based care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Partners with others to effect change and produce optimal outcomes. Supports colleagues through knowledge sharing to provide safe, quality HBPC nursing. Perform other related duties as listed in functional statement.
