About the role

The Women's Health Clinical Navigator is responsible for providing competent, evidence-based care to assigned patients. They provide patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing.

THIS IS NOT A REMOTE POSITION Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinate care with multiple providers across all levels and sites of care. Assure services interfacing within the Veterans Affairs (VA) and community programs provide continuity of care to Veterans. Coordinate and facilitate WH care including contacting the Veteran, facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, and notifying providers of needed follow up care. Interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards. Must pass pre-employment physical evaluation. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
