About the role
As a Care Coordinator, you will be the central link between clients, healthcare professionals, and our dedicated care teams. You will oversee the planning, coordination, and ongoing management of care packages, ensuring outstanding service delivery and excellent communication at every stage.
This is a varied and impactful role where your organisational skills and people-first approach will directly improve the lives of others!
Key Responsibilities
- Coordinating and scheduling care packages to ensure seamless service delivery
- Acting as a key point of contact for clients, families, healthcare professionals, and care staff
- Supporting field staff and responding calmly to changes or challenges
- Maintaining accurate records and managing confidential information professionally
- Always delivering a high standard of customer service
