We're recruiting for an Adult Nurse Case Manager to join a growing specialist complex care provider, overseeing high-quality care packages across Bristol and North Somerset.

You'll have the autonomy to manage your own caseload, provide clinical leadership to dedicated care teams and make a genuine difference to adults with complex healthcare needs in their own homes.
