The Role Were looking for an organised and proactive Care Coordinator to support the delivery of high-quality, person-centred care while helping to grow and develop our services.
This is a fast-paced role requiring strong communication, rota management, and the ability to build relationships with clients, staff, and healthcare professionals.
Key Responsibilities Coordinate care packages from referral through to delivery
Manage and optimise staff rotas (holidays, absence, coverage)
Recruit, onboard, and support care staff
Liaise with clients, carers, and professionals (GPs, Commissioners, Nurses)
Maintain high standards of care, compliance, and service quality
Conduct staff supervisions and support performance management
Support business growth through client relationship management
Participate in an on-call rota
About You Experience in a domiciliary care coordination role (preferred)
Strong organisational and problem-solving xwzovoh skills
Confident communicator with a proactive approach
Experience with rostering systems (e.g. Access Planner desirable)
Able to manage multiple priorities in a fast-paced environment
Full UK driving licence (preferred)
