About the role
Community Rehabilitation Support Worker – Elderly Rehabilitation
We are currently recruiting Community Rehabilitation Support Workers to join our 7-day Elderly Rehabilitation Service across Nottingham.
This is an excellent opportunity for caring, reliable, and experienced individuals who are passionate about supporting older adults to regain independence and remain safely in their own homes.
Working Pattern
This is a 7-day service (Monday to Sunday) with a variety of shift patterns, including:
* Early: 07:00 – 14:00
* Late: 14:00 – 22:00
* Long Day: 07:00 – 22:00
Flexible shifts may also be available, including 07:00 – 19:00, depending on service needs.
Pay Rates
* Weekdays: £14.00 – £14.50 per hour
* Nights & Saturdays: £18.00 per hour
* Sundays & Bank Holidays: £22.00 per hour
Key Responsibilities
* Visit patients in their own homes across Nottingham City.
* Support elderly patients with rehabilitation and promoting independence.
* Provide personal care where required.
* Assist with safe moving and handling.
* Follow individual clinical care plans accurately.
* Monitor and report any changes in patients' wellbeing.
* Work collaboratively with healthcare professionals to deliver high-quality care.
* Maintain accurate documentation and adhere to safeguarding procedures.
Essential Requirements
Applicants must:
* Hold a full UK driving licence.
* Have access to a reliable vehicle for travelling between patient visits.
* Have previous experience supporting elderly individuals in a health or social care setting.
* Be competent in:
* Moving and Handling
* Personal Care
* Following Clinical Care Plans
* Basic Life Support (BLS)
* Safeguarding
* Have excellent communication skills and a compassionate, patient-centred approach.
* Be reliable, flexible, and able to work independently within the community.
Preferred Candidates
We are particularly interested in applicants who are local to Nottingham or the surrounding area, as previous experience has shown that locally based staff are better suited to the travel requirements of this community role.
