Job DescriptionWhat youll do?
Every day as a HICSS Care Assistant is different, but your impact is always the same helping people recover, regain independence, and remain safely at home.
You will:
- Support individuals following hospital discharge or during periods of recovery
- Deliver person-centred care in line with care plans and clinical guidance
- Assist with personal care while promoting dignity and independence
- Monitor wellbeing and report changes to the wider multidisciplinary team
- Support mobility, medication, nutrition, and daily living activities
- Work closely with healthcare professionals as part of an integrated local team
What we are looking for?
HICSS roles support individuals with more complex needs, so were looking for people who combine compassion with capability. Whether you already have experience in care or are looking to build on a solid foundation, youll play a key role in supporting recovery and independence at home.
Were looking for people who:
- Have previous care experience, ideally within community, reablement, or hospital discharge settings
- Hold (or are willing to work towards) a relevant qualification such as a Care Certificate or NVQ/QCF Level 2 in Health & Social Care
- Are confident following care plans and working alongside healthcare professionals
- Are observant and able to recognise and report changes in condition
- Are reliable, committed, and able to work independently in the community
- Have a full UK driving licence and access to your own vehicle
