About the Role
We are recruiting a Band 6 Community Dietitian to join the Community Adult Rehabilitation Service within CNWL.
This is an excellent opportunity to work as part of a supportive multidisciplinary team, delivering specialist nutritional support to adults within their own homes, care homes, and intermediate care settings.
Key Responsibilities

Provide specialist dietetic assessment and treatment for adults requiring nutritional support, including Home Enteral Tube Feeding (HETF)

Deliver evidence-based nutritional interventions for patients who are malnourished, at risk of malnutrition, or enterally fed
-
Assess and calculate nutritional requirements, including food fortification, oral nutritional supplements, and enteral feeding regimens
-
Provide dietary counselling and support behaviour change to patients, families, and carers
-
Make recommendations to GPs regarding appropriate prescribing of oral nutritional supplements and enteral feeds
-
Participate in MDT meetings, discharge planning, and collaborative community care pathways

Maintain accurate clinical documentation and caseload management

Support training and education for healthcare staff, carers, and students

Contribute to audit, service development, and quality improvement initiatives
Essential Requirements
-
Degree or postgraduate qualification in Dietetics
-
HCPC registration as a Dietitian
-
Experience working within community or adult rehabilitation settings
-
Knowledge of nutritional support, including oral nutrition support and enteral feeding
-
Strong communication, clinical reasoning, and organisational skills
-
Ability to work independently and manage a clinical caseload effectively
-
Full UK driving licence and access to a vehicle for community visits
Desirable
-
Experience in Home Enteral Tube Feeding services
-
Experience supervising students or support staff
-
Knowledge of rehabilitation and long-term condition management
-
Experience contributing to audit or service development projects
