About the RoleAs part of our outreach team, you will: Provide personalised, community-based support to individuals with complex mental health needs, promoting independence within their own homes. Work closely with clients to implement and review tailored care plans, ensuring their safety and well-being. Collaborate with multidisciplinary teams, including clinical professionals, to deliver trauma-informed and recovery-focused care.
