Job summaryBrook Lane Surgery is a friendly, patient-focused GP practice serving our local community in Hampshire. We are looking for a motivated and compassionate Care Navigator to join our busy and supportive team.As a Care Navigator, you will play a vital role in ensuring patients receive the right care, at the right time, from the right service.
