Are you an empathetic, compassionate and dynamic individual who wants to make a difference in your role?
Are you happy to travel in your role?
Russell House is a well-established service based in Newark-On-Trent providing accommodation and support for homeless people often with a range of complex support needs. It is a 15-bed hostel with 30 dispersed flats in the community.
We are looking for a reliable, enthusiastic and self-motivated Support Planner who can demonstrate an understanding of the issues facing those affected by homelessness.

To ensure that tasks identified in support plans are communicated to Support Workers and that these tasks are clear, appropriate and agreed with the Service User
You will have caseloads with Tenant's in properties across the Newark and Sherwood (mileage system for car drivers).
To prepare agreed support plans with Service Users.
Sign post Tenant's onto appropriate services for substance misuse management & health. •
Assist service users with housing applications and move on options.
Assist service users with basic budgeting and life skills in order to support them to move on to live independently.
To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
