Transition Practice Lead Surrey | £36,000 | Full Time | Permanent Are you an experienced SEND professional looking for an opportunity to lead and shape outstanding transition support for learners with additional needs? We are seeking a Transition Practice Lead to oversee and develop high-quality transition processes for learners with SEND, EHCPs and other barriers to learning. The Role You will lead transition, assessment and admissions processes, ensuring learners receive the right support from day one. You will also: Coordinate and quality assure EHCP consultations and statutory processes Develop effective transition and learner support plans Lead and support a team of Transition Officers Work closely with Local Authorities, schools, SENCOs, parents/carers and external partners Support EHCP annual reviews and ensure compliance with the SEND Code of Practice Monitor learner needs, provision and High Needs funding Drive high-quality, inclusive practice across the organisation About You We're looking for someone with: Experience working with SEND learners in an education or relevant support setting Knowledge of EHCPs, SEND transition and the SEND Code of Practice Experience coordinating EHCP consultations or statutory processes Previous line management/team management experience Experience working with Local Authorities, schools, SENCOs and families Strong communication, organisation and relationship...
