We're working with a forward‑thinking local authority in Beverley looking for a Transitions Social Worker to support young people as they move into adulthood - combining children's and adult disability services in a truly rewarding role. 🔍 About the Role This is a fantastic opportunity to work within a specialist Transitions Team, supporting young people with disabilities as they prepare for adulthood, while also liaising closely with adult services.
