About the role
Support Worker
This role is responsible for the service delivery of an effective, high quality, person centred support service to residents with a background of rough sleeping and range of medium to high needs. These includes entrenched drug and/or alcohol issues, mental health diagnosis and a range of offending backgrounds.
They will be the principal providers of support to residents in the Harbour Project team and will also work alongside Assistant Support Workers, to promote resident engagement through the development of positive relationships with our residents and contributing towards the promotion of a fulfilling, safe and inclusive environment.
Support Worker Responsibilities:
* Deliver high-quality support to residents within the Harbour Project team.
* Build positive and professional relationships with residents.
* Carry out assessments, support plans, and risk assessments.
* Review and update support plans regularly.
* Support residents with immediate practical and emotional needs.
* Encourage engagement in meaningful activities and support services.
* Use trauma-informed and motivational approaches when working with residents.
* Respond to referrals and assess eligibility for the service.
* Maintain accurate case notes and records on the case management system.
* Monitor outcomes and collect follow-up information where required.
* Work collaboratively with external agencies including police, social services, and healthcare providers.
* Promote resident safety and develop risk management strategies.
* Attend team meetings, reviews, and training sessions.
* Maintain high standards of customer service and professionalism.
* Ensure all work complies with organisational policies and procedures.
* Keep up to date with housing, welfare, and safeguarding legislation.
* Support a positive team environment and work collaboratively with colleagues.
* Follow health and safety procedures within the workplace.
* Undertake additional duties appropriate to the role when required.
Requirements
* Experience supporting vulnerable adults or individuals with complex needs.
* Understanding of substance misuse, mental health, and supported housing.
* Knowledge of safeguarding and health & safety practices.
* Strong communication and relationship-building skills.
* Ability to manage challenging behaviour using de-escalation techniques.
* Good IT skills, including Microsoft Office and case management systems.
* Ability to work independently and as part of a team.
* Flexible to work shifts, including evenings, weekends, and waking nights.
* Organised, reliable, and able to manage competing priorities.
* Must pass a DBS check and have the right to work in the UK.
