(including Bank Holiday)Work MobileWork LaptopExcellent Pension SchemeCompany Car Available (preferred own car)Training Provided Ongoing Training SupportAbout the RoleAs our Complex Care Coordinator, youll be the hub of daily care deliveryrostering visits, supporting care staff, liaising with health and social care professionals, and keeping real-time oversight of calls and records. This is a hands-on role where your attention to detail makes a real difference to peoples lives.What youll be doingBuilding accurate roster and allocating visits in line with care plans, risk assessments, continuity and preferencesMonitoring calls on Access and responding quickly to missed, late or short callsCoordinating same-day cover for sickness and emergencies and communicating changes clearlyChecking daily care logs and medication records/MARs, escalating issues immediatelySupporting quality and compliance through record checks, spot checks and staff supervisionLiaising with GPs, District Nurses, pharmacies, social workers and Local Authority teamsEscalating safeguarding concerns promptly and following local proceduresHelping with assessments, care plan updates, reviews and settling-in checks for new packagesWorking Pattern Options:Full timeOffice and community-basedIncludes participation in an on-call roster.What were looking for (essential)Experience coordinating domiciliary/home care (or a similar operational role) - Minimum 2 years experience.Confident IT skills (Microsoft 365 +Access rota system, e-rostering/e-call monitoring systems such as Access)Knowledge of CQC Fundamental Standards / relevant domiciliary care requirementsStrong organisation, communication and time-managementable to work under pressureNVQ/QCF Level 2+ in Health & Social CareExperience supervising staff, rota management and completing spot checks/supervisionsWhy join Chenai Holistic Home Care Agency Ltd?Youll be joining a supportive team built around our values of Care, Respect, Trust, Quality and Teamworkwhere doing the right thing for clients and colleagues matters every day.TPBN1_UKTJ
