Somerset Council is seeking a dedicated and passionate Social Worker to join a supportive multi-disciplinary team based in Shepton Mallet. This is an excellent opportunity for a Social Work England registered professional who is committed to delivering positive outcomes for vulnerable adults and working collaboratively with partner agencies. Key Responsibilities * Manage a varied caseload within Adult Social Care. * Undertake strengths-based assessments and care planning. * Work collaboratively within a multi-disciplinary team environment. * Complete safeguarding enquiries and risk assessments. * Develop effective support plans to promote independence and wellbeing. * Liaise with Health Services, Education, Voluntary Organisations and partner agencies. * Maintain accurate case records and reports using electronic systems. * Ensure practice remains compliant with relevant legislation, policies and Social Work England standards.
