Band 6 Community Psychiatric Nurse (CPN) Location: Essex Job Type: 12-Week Placement Employer: Cromwell Medical Staffing Working Hours: 09:00-17:00 (Monday–Friday) Pay Rate: 26 pounds/hr on Umbrella Job Description Cromwell Medical Staffing is currently seeking an experienced Band 6 RMN to support community mental health services on a 12-week placement. This is an excellent opportunity for a skilled mental health nurse to manage a defined caseload, deliver high-quality, patient-centred care, and contribute to effective community-based mental health interventions. Key Responsibilities * Assess, plan, implement, and evaluate care for a caseload of service users experiencing a range of mental health conditions * Undertake comprehensive mental health and risk assessments, ensuring appropriate risk management plans are in place * Act as Care Coordinator in line with CPA (Care Programme Approach) requirements * Develop and deliver person-centred care plans in collaboration with service users, carers, and the multidisciplinary team * Participate in the Duty Worker and Initial Assessment rota, providing timely assessments and interventions * Support safe and effective discharge planning from inpatient services and facilitate smooth transfers of care.
