Key Responsibilities: Deliver high-quality nursing care in the community, empowering patients to maintain independence. Assess, plan, implement, and evaluate person-centred care plans. Collaborate with multidisciplinary teams, GP surgeries, and acute trusts. Assist in facilitating smooth hospital discharges and reducing readmissions.Clinical skill set required:​ Complex Wound care including Doppler compression Simple wound care End of life care Medication administration Catheterisation male female suprapubic Bowel care Syringe driver Insulin administration Doppler assessment Compression bandagingEssential Qualifications: To apply, you must have: NMC registration (valid and active). A minimum of 6 months of NHS experience as a Community Nurse.
