OUTREACH FAMILY SUPPORT WORKER Directorate: Children & Young People’s Services Division: Early Help Job Title: Outreach Family Support Worker Grade: 5 Salary: £23,836 – £27,905 Location: Liverpool DBS: Enhanced DBS Reports to: Early Help Consultant Social Worker Job Purpose To provide high-quality, targeted outreach and home-based Early Help support to children, young people and families using a whole-family approach. The role will support children and young people aged 0–25* and their families, helping to prevent escalation to statutory services and ensuring families can access appropriate universal and community services. *Ages 19–25 applies where the SEND Code of Practice is applicable. Key Responsibilities Hold complex Level 3 cases stepped down from Children’s Social Care. Complete Early Help Assessments and coordinate Team Around the Family (TAF) meetings. Act as Lead Professional, coordinating services and monitoring progress against family plans. Provide intensive, targeted and bespoke support to vulnerable children, young people and families. Support adolescents experiencing complex emotional and mental health difficulties. Assess risk and need and develop plans to reduce identified risks. Work with families using Signs of Safety, solution-focused approaches and the Family Outcomes Star. Develop practical parenting strategies and strengthen parent/carer-child relationships. Carry out home visits and provide direct support within the family environment. Use de-escalation techniques when working with challenging behaviour. Support supervised contact where required. Maintain accurate case records, assessments, chronologies, plans and progress notes. Promote safeguarding and equality while working in partnership with children, young people, families and professionals. Attend supervision, reviews, planning meetings and relevant multi-agency forums. Contribute to service development, professional learning and continuous improvement. Ideal Candidate The successful candidate will have experience of: Working with children, young people and families in an Early Help or family support setting. Managing complex cases and assessing risk and need. Delivering intensive, targeted and home-based support. Coordinating multi-agency professionals around families. Developing and reviewing family support plans. Safeguarding children and young people. Supporting positive parenting and family relationships. Engaging families using a strengths-based, solution-focused approach. Enhanced DBS required
