Our Client, a Greater Manchester council, is seeking an Adult's Social Worker to join their Integrated Care Team.

Responsibilities of this role:

• Undertake strengths-based assessments under the Care Act 2014
• Develop and review person-centred support and care plans
• Work collaboratively within multidisciplinary integrated care teams
• Coordinate care with health professionals, voluntary sector, and community services
• Manage a varied caseload, including complex and high-risk cases
• Safeguard adults at risk in line with statutory duties and local procedures
• Support hospital discharge planning and prevent unnecessary admissions
• Promote independence, choice, and wellbeing for service users
• Complete mental capacity assessments and best interest decisions
• Maintain accurate, timely case records and statutory documentation

Requirements of this role

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have minimum of 3 year's experience working as a Qualified social worker
