Working Hours: Monday to Friday, 8:30am - 3:30pm
About the School:
A specialist SEND provision in Rochdale, committed to supporting children and young people whose social, emotional, and mental health needs impact their engagement with education. Through a trauma-informed and relationship-centred approach, we create a safe and supportive environment where learners can rebuild confidence, regulate emotions, develop social skills, and achieve both personal and academic success.
We are seeking a committed Youth & Transition Support Mentor. In this role, you will work alongside learners of all ages, supporting them with tailored programmes that enhance independence, employability, community engagement, and personal wellbeing.
You will collaborate with multidisciplinary teams to deliver effective transition planning, life-skills coaching, and opportunities for learners to access education, training, employment pathways, and community experiences.
Youth & Transition Support Mentor Role and Responsibilities:
Support pupils to develop independence, life skills, and employability, including college and vocational transitions.
Work closely with teaching staff, families, and external agencies to create personalised pathways that meet each pupil's individual needs.
Provide guidance, mentorship, and practical support during key transitions, including moving to further education, training, or work placements.
Build positive, trusting relationships with pupils, encouraging social, emotional, and personal development.
Youth & Transition Support Mentor - What We're Looking For:
Experience working with young people with SEND, ideally in a school, residential, or community setting. A calm, empathetic, and proactive approach to managing behaviour
Strong communication and relationship-building skills
A commitment to helping pupils achieve independence and life success.
A relevant qualification in support, care, or education is desirable but not essential
Youth & Transition Support Mentor - To Be Eligible You Must:
Hold the Right to Work in the UK
Have an enhanced child barred list DBS on the update service (or be willing to apply)
Be able to provide two professional references (child-related preferred)
