Responsibilities of this role:
- Support adults aged 65+ to achieve safe, timely hospital discharges and successful transitions back into the community.
- Complete strengths-based assessments and identify appropriate discharge pathways that promote independence and wellbeing.
- Work collaboratively with health professionals, families, carers, and partner agencies to coordinate discharge plans and resolve barriers.
- Facilitate person-centred interventions using a "What Matters" approach to maximise recovery and prevent unnecessary hospital stays.
- Apply relevant legislation, including the Mental Capacity Act, Mental Health Act, and Social Services and Wellbeing (Wales) Act, within practice.
- Contribute to safeguarding, risk management, and care planning to ensure positive outcomes for vulnerable adults.
- Act as a key liaison between hospital teams, adult social care, and community services to support integrated working.
- Promote effective discharge planning and service coordination to reduce delays and improve patient outcomes.
