Hackney's Reablement at Home service supports adults to regain independence and achieve therapy-led goals in their own homes. The service takes a reablement-first approach, focusing on prevention, recovery and reducing reliance on long-term care through short-term, structured interventions.
As an Occupational Therapist within this service, you will play a pivotal role in delivering strengths-based, person-centred reablement, helping residents rebuild skills, confidence and control over their daily lives.
Key ResponsibilitiesWorking closely with reablement providers, Senior Practitioners and Occupational Therapy Assistants, you will:
- Complete comprehensive OT assessments within the community
- Identify personalised outcomes and reablement goals with service users
- Develop and issue Reablement Therapy Plans at the point of referral
- Monitor and review progress throughout the reablement period
- Provide professional guidance and consultation to reablement providers
- Apply a strengths-based, preventative approach to all interventions
- Support improved flow through hospital discharge pathways
- Ensure support is risk assessed, proportionate and outcome focused
- Maintain accurate, timely case records in line with statutory frameworks
This role is based at Hackney Service Centre and involves frequent home visits across the borough.
About YouYou will be an experienced Occupational Therapist with a strong interest in reablement, prevention and community-based practice.
Essential requirements include:
- A UK-recognised degree in Occupational Therapy
- HCPC registration
- Experience working in community or reablement settings
- Sound knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act
- Confidence applying strengths-based and person-centred approaches
- Ability to work effectively within multidisciplinary teams
- Strong communication, assessment and clinical reasoning skills
- Meaningful, outcome-focused work supporting independence
- A role embedded in preventative adult social care practice
- Opportunities to work collaboratively across health and social care
