Job title: Adult Social Worker - Hospital Team
Role purpose:
To provide professional social work support to adults in hospital, helping patients and their families address social care, safeguarding, wellbeing and discharge needs. The role involves working closely with health and social care professionals to ensure patients can leave hospital safely and with appropriate support.
Key responsibilities
Complete assessments of adults' social care and support needs.
Develop and review care and support plans.
Support safe and timely hospital discharge.
Assess patients' ability to manage safely at home and identify appropriate support.
Arrange or coordinate care packages, residential or nursing placements and community support where appropriate.
Work with patients, families, carers and other professionals to identify solutions.
Undertake safeguarding assessments and respond to concerns about abuse or neglect.
Promote independence, choice, dignity and person-centred care.
Participate in multidisciplinary team meetings and discharge planning.
Liaise with hospitals, local authorities, care providers, community teams and voluntary organisations.
Maintain accurate and confidential records in accordance with professional and organisational requirements.
Complete assessments and make appropriate professional decisions in line with relevant legislation and local policies.
Manage a caseload effectively and prioritise urgent or high-risk situations.
Provide information, advice and advocacy to patients and their families.
Contribute to improving services and supporting positive outcomes for adults.
