Working independently you will provide a high quality of service to patients
and their families, ensuring their care and treatment needs are met.You will need to have ACP experience of working with adults with minor illness, long
terms conditions and complex health needs. You will be exceptionally organised with a
high-level of attention to detail. You will naturally possess excellent
inter-personal skills, and an ability to consult and positively engage with key
stakeholders across the organisation.Job DescriptionYou will work as a
Bank ACP and ensure the safe and effective delivery of a high standard
of individualised care across flexible sessions Monday to Saturday, and where required in collaboration with the Primary care Multidisciplinary
Team.Part of the EA role will require some Home Visiting Consultations.You will work
