Job Purpose
To provide high-quality social work support to young adults aged 18-25 experiencing mental health challenges. The post holder will assess needs, manage risk, promote independence, and work collaboratively with individuals, families, carers, and partner agencies to improve well being and achieve positive outcomes.
Key Responsibilities
- Undertake comprehensive social care and mental health assessments in line with relevant legislation and professional standards.
- Develop, implement, and review person-centred care and support plans.
- Build effective relationships with young adults to empower them in making informed decisions about their lives and wellbeing.
- Safeguard vulnerable adults and manage risks through appropriate interventions and multi-agency working.
- Support young adults transitioning from children's services into adult mental health services.
- Work collaboratively with health professionals, education providers, housing services, employers, and community organisations.
- Provide information, advice, advocacy, and signposting to relevant support services.
- Maintain accurate, timely, and detailed case records using approved systems.
- Participate in multidisciplinary meetings, case conferences, and safeguarding investigations.
- Support individuals in accessing education, employment, training, and community opportunities.
- Promote recovery-focused and strengths-based approaches to practice.
- Ensure compliance with relevant legislation, policies, procedures, and professional standards.
Experience
- Experience of working with young adults or adults with mental health needs.
- Experience of undertaking assessments, care planning, and risk management.
- Experience of multi-agency and multidisciplinary working.
- Knowledge of safeguarding adults' procedures and practices.
Knowledge
- Understanding of the Care Act 2014 and Mental Capacity Act 2005.
- Knowledge of mental health conditions and recovery-focused practice.
- Understanding of strengths-based and person-centred approaches.
- Knowledge of safeguarding legislation and professional responsibilities.
