The Role We are looking for a compassionate, organised and proactive Pathway Coordinator to join our dedicated team in Seaforth, Liverpool.
This is an opportunity to make a genuine difference every single day by supporting people on their journey towards independent living and creating clear pathways into long-term accommodation and stability.
You will deliver structured, person-centred support to individuals within supported housing, helping them progress through tailored support plans, move-on pathways and practical life skills development.
Key Responsibilities Deliver person-centred support and progression plans
Support individuals towards independent living and move-on accommodation
Carry out support planning, reviews and risk assessments
Support access to training, employment and life skills opportunities
Work collaboratively with external agencies and partners
Maintain accurate case records and support documentation
Promote safeguarding, wellbeing and positive community engagement
Support housing management responsibilities where required
About You We would welcome applications from candidates with experience in:
Supported housing
Homelessness services
Mental health or substance misuse support
Safeguarding and risk management
Person-centred support planning
You will be empathetic, resilient, organised and passionate about supporting people to achieve lasting independence.
