Our client, a Greater Manchester council is seeking an Adults Social Worker to join their LD Team.

Responsibilities of this role:

• Undertake Care Act assessments, reviews, risk assessments and support planning for adults with learning disabilities.
• Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.
• Manage a caseload of adults with complex needs, promoting independence, choice and wellbeing.
• Work collaboratively with families, carers, health professionals, advocacy services and community partners.
• Coordinate and review care packages, supported living placements and day opportunities to ensure needs are met.
• Safeguard vulnerable adults by identifying risks and undertaking safeguarding enquiries where required.
• Maintain accurate case records, assessments and reports in line with statutory duties and local authority procedures.
• Support service users through transitions, life changes and long-term planning using a strengths-based approach.

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
