About the Role An exciting opportunity has arisen for an experienced Practice Nurse to join a well-established, patient-focused GP practice serving approximately 10,500 patients in Halifax.
This forward-thinking practice is seeking a motivated and enthusiastic nurse with previous General Practice experience to join its friendly and supportive clinical team. The successful candidate will play a key role in delivering high-quality patient care while contributing to the ongoing development of services within the practice.
This is an excellent opportunity for a Practice Nurse looking to further develop their skills within a supportive environment that values professional development, teamwork, and innovation.
Key Responsibilities As a Practice Nurse, you will be responsible for:
? Chronic Disease Management (Diabetes, Asthma & COPD)
? Cervical Cytology (Smears)
? Childhood & Adult Immunisations
? Wound Care & Dressings
? Contraceptive Advice & Support
? NHS Health Checks & Screening Programmes
? Health Promotion & Lifestyle Advice
? Infection Prevention & Control
? Supporting the Practice in achieving QOF, IIF and Quality Targets
? Supporting the development of Nurses and Healthcare Assistants where appropriate

Essential Requirements ? Registered Nurse (NMC Registered)
? Previous experience working as a Practice Nurse within a GP Surgery
? Experience managing long-term conditions such as Diabetes, Asthma and COPD
? Competent in Cervical Cytology (Smears)
? Experience delivering Childhood & Adult Immunisations
? Strong communication and teamwork skills
? Evidence of ongoing professional development
? Competent using SystmOne
Desirable Skills ? Independent Nurse Prescriber Qualification
? Additional qualifications in Chronic Disease Management
? Experience in Minor Illness Clinics
? Triage Experience
