Were recruiting for aHospital Discharge Social Workerto join a supportive local authority team. This is an opportunity to work in a fast-paced, collaborative environment while benefiting from a flexible hybrid model, combining hospital, community and home-based working.
Youll work alongside NHS colleagues, ward teams, therapists, community providers and families to help ensure safe, person-centred and timely discharges from acute settings.

The role
As a Hospital Discharge Social Worker, youll play a key part in ensuring people leave hospital safely, with the right level of care and support in place.
Your responsibilities will include:
Completing timely, strengths-basedCare Act assessments
Supporting discharge planning from acute hospital and community settings
Working closely with multidisciplinary teams to coordinate safe discharge arrangements
CompletingMental Capacity Assessmentsand contributing to Best Interest decisions where required
Arranging care packages, reablement and community-based support
Promoting independence and helping to prevent avoidable readmissions
Liaising with individuals, families, advocates and external professionals throughout the discharge process
To be considered, you will need:
A recognisedSocial xwzovoh Work qualification
CurrentSocial Work England registration
Experience inHospital Discharge, Discharge to Assess, Adult Social Care or Adult Community Services
Strong knowledge of theCare Act,Mental Capacity Actand strengths-based practice
Confidence managing complex cases in a busy multidisciplinary setting
Experience using Mosaic, or another case management system, would be beneficial
