This is an excellent opportunity for a practitioner who is passionate about promoting independence, delivering person-centred care, and supporting vulnerable adults to achieve positive outcomes within their local communities. The role is community-based, with a strong emphasis on enabling individuals to remain safely and independently within their own homes wherever possible. Key Responsibilities Undertake comprehensive assessments and fulfil statutory responsibilities under the Care Act 2014. Manage and support a caseload of adults with disabilities, older people, and their carers. Contribute to Multi-Agency Protection of Vulnerable Adults procedures and safeguarding investigations.
