Adults Social Worker - Dementia Team

Are you an experienced Qualified Social Worker looking to make a real impact supporting older people living with dementia? I'm working with a well-established Adult Social Care team in Newcastle who are looking for someone who can step straight into a busy, rewarding role supporting vulnerable adults and their families. 🔑 The Role You will be working with older adults affected by dementia, completing person-centred assessments, developing care plans, and working closely with families and partner agencies to deliver the right support at the right time.
