Hub Pharmacist - Clinical Pharmacist and Independent Prescriber

Job summaryThe successful candidate will work in our pharmacy hub across all of our established
sites, offering exposure to
enhanced services and modern models of care delivery.We are looking for a proactive and enthusiastic Pharmacist
with prescribing qualifications, preferably with Chronic care or minor illness
experience, to join our Practice to work closely with our nurses and GPs.The appointed Pharmacist would be part of a wider pharmacy
team working with our associate practices within a Hub model.We appreciate the interest of all applicants in this

responsibility for areas of Chronic Disease Management and undertake Clinical
Medication Reviews, proactively manage patients with complex poly-pharmacy
needs. The role would also entail providing primary support to GP staff with
regards to prescriptions (this includes repeat prescribing), medication
queries, complex clinical queries - please see full job description. There will
also be training to manage low-level investigation results following strict
protocol and supervision.We will look to provide support to develop and train the
candidate to further their skills in their clinical interests. The Pharmacist
would have an allocated Lead Pharmacist and GP Partner to mentor, support and
