Are you an experienced Social Worker looking for a rewarding Hospital Discharge role where you can directly support people to regain independence following a hospital stay? We’re recruiting for a Hospital Discharge Social Worker to join a supportive local authority team. You’ll work alongside NHS colleagues, ward teams, therapists, community providers and families to help ensure safe, person-centred and timely discharges from acute settings. Your responsibilities will include: Completing timely, strengths-based Care Act assessments Supporting discharge planning from acute hospital and community settings Working closely with multidisciplinary teams to coordinate safe discharge arrangements Completing Mental Capacity Assessments and contributing to Best Interest decisions where required Arranging care packages, reablement and community-based support Promoting independence and helping to prevent avoidable readmissions Liaising with individuals, families, advocates and external professionals throughout the discharge process To be considered, you will need: A recognised Social Work qualification Current Social Work England registration Experience in Hospital Discharge, Discharge to Assess, Adult Social Care or Adult Community Services Strong knowledge of the Care Act, Mental Capacity Act and strengths-based practice Confidence managing complex cases in a busy multidisciplinary setting Experience using Mosaic, or another case management system, would be beneficial If you’re looking for a meaningful role where you can make a genuine difference to adults and families at a crucial point in their care journey, we’d love to hear from you.
