Adult Social Worker – Preparing for Adulthood Service Care Solutions | Thurrock Council Hours: 37 hours per week Location: Hybrid – expectation of attending the office twice per week (or as per service requirements) Pay Rates: £28.90 per hour – Umbrella £25.49 per hour – PAYE (inclusive) £22.74 per hour – PAYE (exclusive)Contract: Initially 3 months Job Overview Service Care Solutions are recruiting for an experienced Adult Social Worker to join Thurrock Council’s Preparing for Adulthood Team. This is an agency role, suitable for a qualified Social Worker with experience supporting young people transitioning from Children’s to Adult Social Care. You will work with young adults with learning disabilities and impairments, supporting them through person-centred, strength-based assessments and care planning in line with the Care Act 2014. You will be joining a stable, award-winning and innovative service that promotes professional development and collaborative working. Key Responsibilities Complete strength-based assessments and reviews in line with the Care Act 2014 Work directly with young adults and families to identify needs, wishes, and outcomes Support individuals experiencing crisis or at risk of crisis Develop outcome-focused care plans and commissioning plans Undertake Mental Capacity Assessments in line with the Mental Capacity Act Complete Continuing Health Care (CHC) Checklists Identify safeguarding concerns and follow inter-agency safeguarding procedures Produce high-quality court reports and written reports within timescales Work collaboratively with Children’s Services, Health, Education, and voluntary organisations Maintain accurate and timely case records Attend supervision, training, and contribute to service developmentValues & Professional Expectations You will be expected to work in line with Thurrock Council’s core values: Ambitious Collaborative Compassionate Empowered Proud InclusiveYou must: Work in an anti-discriminatory and person-centred manner Promote equality, diversity, and inclusion Comply with data protection, health and safety, and safeguarding policies Work flexibly, including remote and office-based working as requiredRequirements Recognised Social Work qualification (Degree / DipSW) Social Work England registration Experience working with young people transitioning to Adult Services Strong knowledge of the Care Act 2014, MCA, and safeguarding Excellent communication and report-writing skillsService Care Solutions benefits and interested please send your CV to (url removed)
