This At Home Service – Integrated Care Social Worker role will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance.   At Home Service – Integrated Care Social Worker Responsibilities include: To represent my team in triaging and collaborating with other professionals like the OT and doctors to triage cases appropriately. ⁠To represent my team in MDT meetings to update the whole triaging team about the current state of cases that has been assessed and accepted as requiring social care intervention. ⁠To complete Care Act assessment and review of needs to identify the needs of our service-user and provide the right interventions. Example, packages of care. ⁠To make referrals to other Agency for support.
