Preparing for Adulthood Social Worker

This will include supporting planning around education, employment, health, housing, independence and community inclusion. What you’ll need  A recognised Social Work qualification Current Social Work England registration Experience supporting young people with disabilities, additional needs or complex needs Knowledge of transition planning and preparing for adulthood pathways Strong assessment, care planning and partnership-working skills A person-centred, strengths-based approach to practice Why work with Leaders in Care?
