Support Worker
Newport | Up to £13.44 per hour | Days/Nights | Shift Work | Permanent
Introduction
Acorn by Synergie are recruiting dedicated and compassionate Support Workers to support an individual with mental health needs and a physical disability in their own home. This is a person-centred role focused on promoting independence, maintaining routines, and supporting safe engagement within the community.
Key Duties:
Deliver high-quality, person-centred care in line with individual support plans.
Monitor mental and physical health, emotional wellbeing, and overall presentation.
Support with all Activities of Daily Living (ADLs).
Promote independence and a structured, meaningful daily routine.
Encourage engagement in purposeful and enjoyable activities.
Support access to the community, appointments, and social opportunities.
Assist with faith-based routines and religious practices.
Safely drive the individual's vehicle where required.
Maintain accurate and up-to-date records in line with regulatory standards.
Uphold safeguarding responsibilities and maintain professional boundaries.
Requirements:
Registration with, or willingness to register with, Social Care Wales.
Enhanced DBS check.
Right to work in the UK.
Full UK driving licence.
Values-based approach with strong commitment to person-centred care.
Strong communication skills and ability to work as part of a consistent team.
Previous experience supporting individuals with mental health needs or physical disabilities (preferred).
Flexibility to work a range of shifts, including days, nights, and sleep-ins.
Willingness to complete all mandatory training.
