You will work closely with health professionals, service users, families and other agencies to support safe, timely and appropriate hospital discharges.Key Responsibilities

• Undertake complex Care Act assessments, reviews and support planning.
• Manage hospital discharge cases, ensuring individuals are discharged safely and appropriately.
• Work collaboratively with NHS colleagues, OTs, health professionals, care providers and families.
• Assess care and support needs and identify appropriate packages of care, reablement and community support.
• Manage safeguarding concerns and complex cases where required.
• Apply professional judgement when managing risk and making decisions around discharge.
• Maintain accurate and timely case records in line with statutory requirements.
