Our client Blackburn with Darwen Council is looking for an Adults Social Worker to join their Hospital/Discharge to assess Team.
Job description:
We are looking for a skilled and motivated Social Worker to join our Hospital Discharge and Discharge to Assess (D2A) service. This is a dynamic, fast-paced role focused on supporting timely, safe, and person-centred discharges from hospital into community settings via Discharge to Assess pathways. ASAP Start
Working as part of a multi-disciplinary team, you will play a key role in ensuring individuals leave hospital with the right support in place, enabling them to recover and regain independence in the most appropriate environment.
This is a hybrid role, offering a balance of office based ,on-site hospital work, community visits, and remote working.

Key Responsibilities
Undertake strengths-based assessments in line with Care Act principles
Facilitate safe and timely hospital discharges through the D2A pathway
Work collaboratively with health professionals, families, and community services
Promote independence, wellbeing, and risk enablement
Develop and review support plans that reflect individual goals and outcomes
Safeguard vulnerable adults and manage risk appropriately
Maintain accurate and timely case recordsAbout You
Qualified Social Worker with current Social Work England registration
Experience in adult social care, ideally within hospital discharge or community settings
Strong understanding of strengths-based and person-centred practice
Ability to work in a fast-paced environment and manage competing priorities
Excellent communication and partnership-working skills
Confident decision-maker with a proactive and solution-focused approach
