Richmond and Wandsworth - OT - Reablement Service £32PH

Eden Brown Synergy are currently recruiting for an Occupational Therapist to join the Reablement Service within Richmond and Wandsworth Councils. This is a fantastic opportunity to be part of a dynamic and supportive team focused on promoting independence, improving health, and enhancing wellbeing for adults in the community. The service provides short-term, outcome-focused interventions following referrals from hospitals, community services, and social care teams. Pay Rate: £32 per hour (Umbrella) Key Responsibilities Manage a caseload of adults (18+) requiring assessment, review, and ongoing support Complete home visits and deliver person-centred interventions Promote independence through strengths-based practice Work collaboratively with care agencies, families, and carers Prescribe and recommend adaptive equipment and minor adaptations Complete and update case records on Mosaic system Undertake Moving & Handling risk assessments and develop handling plans Carry out: Care Act Assessments Mental Capacity Assessments Carers Assessments Reablement assessments and reports Support individuals to achieve outcomes and manage risks in daily living Working Pattern 36 hours per week (Monday-Friday) Hybrid working (home working + office base + community visits) Optional weekend overtime available Requirements Degree/Diploma in Occupational Therapy Current HCPC registration Experience within adult social care or reablement services Strong knowledge of the Care Act 2014 and relevant legislation Experience working in multidisciplinary teams Strong assessment, report writing, and organisational skills Ability to manage a busy caseload effectively What's on Offer Supportive team environment with strong supervision Ongoing learning and development (CPD) opportunities Opportunity to make a real impact in the community Benefits of Working with Eden Brown Synergy DBS check processed and paid for Referral bonus for recommending a friend or colleague Bonus for securing your own role Twice weekly payroll Dedicated consultant support throughout your placement How to Apply If you are interested in this position, please contact: Shristi Yadav 📧 📞 (phone number removed) Eden Brown Synergy is an equal opportunities employer.
